Name-Manish Kesharwani, Name of the Father-Jawahar Lal Kesharwani, Application number for the registration of the pharmacists-UPPCR0004441Address- Rani bag, Tathai Road, Mirzapur city, District-Mirzapur, Uttar Pradesh, PIN Code-231001, Mobile number-9369242769Information seeker paid Rs.1550.00 as fee for the registration. on 31 January 2022 quite obvious from the attached payment slip as first page in the attached PDF document to this online R.T.I. application.
