* Prayer or Relief Sought It is quite obvious that district magistrate Mirzapur is the nodal officer of the district under right to Information Act 2005 and the district magistrate should act like facilitator of the facilities provided by the government to the information seekers and district magistrate has delegated its power to chief revenue officer. The matter concerns principal of Maa Vindhyavasini government Medical College as MCH wing of the government women hospital works under the ambit of Maa Vindhyavasini government college Mirzapur and it was obligatory duty of C.R.O. to transfer it to concerned as nodal officer or under subsection 3 of section 6 of the right to Information Act 2005 as public information officer of the collectorate. Because of the rampant corruption in the working provisions of the right to Information Act 2005 are not being followed by the public authorities. Staff of the government are so incompetent to understand the contents of the applications submitted by the information seekers.

State Public Information Officer/ principal of Maa Vindhyavasini government Medical College as MCH wing of the government women hospital works under the ambit of Maa Vindhyavasini government college Mirzapur may provide the following information to the information seeker as follows.
1-According to the available X-Ray report, delivery was to take place in the month of June 2022 and your three doctors in the report found the patient in labour pain in the beginning of the month of April 2022.
Please provide the test carried out by the three doctors as mentioned in the report or any other thing on which findings are based.
2-Please provide the copy of consent of the information seeker in which the information seeker opposed the process of surgery so that the applicant may know when they managed to get a sign of the applicant and how?
3-Please provide the discharge report made on 16 April 2022 when patient was first discharged, and time and name of staff carried out the process as well.
4-Please provide the time when patient was readmitted and name of staff who processed it and time span between discharge and readmission.
5- Please provide the discharge report made on 16 April 2022 finally after the expiration of the patient. After all it is obligatory duty of the caretakers of patients to take discharge reports of the patients, but nothing was provided by the women-hospital Mirzapur.
Please provide aforementioned five points information to the information seeker. Principal of Maa Vindhyavasini government Medical College as MCH wing of the government women hospital works under the ambit of Maa Vindhyavasini government college Mirzapur may provide the 5 point information within stipulated 30 days as prescribed under sub section 1 of section 7 of The Right to Information act 2005.

* Description of Information Sought The matter concerns treatment of Rubi 28 years old W/O Pritesh Kumar resident of Mohalla-Danghar, Bathua, Pathrahia Road, Kotwali Katra, District-Mirzapur admitted in women hospital on 16 April 2022 at 11:25 in the morning who died because of dereliction of doctors and mismanagement in the hospital but later matter was closed by taking cryptic help of MCH wing of women hospital which is hub of corruption.

रूबी उम्र 28 वर्ष पत्नी प्रितेश कुमार निवासी बथुआ डगहर थाना कोतवाली सीटी मीरजापुर दिनांक 16.04.2022 को पेट दर्द कि शिकायत होने पर सुबह 11:25 बजे महिला चिकित्सालय में भर्ती हुयी। जिसे डा० रितु सिन्हा, डा० करिश्मा एवं डा० रविन्द्र द्वारा गहन चिकित्सीय परीक्षण के उपरान्त यह पाया गया कि मरीज लेबर पेन में आयी थी What is the loss if the principal may provide the report which ought to be provided to the applicant as obligatory duty of the principal of Maa Vindhyavasini government medical college but not provided under transparency act after the transfer of application to principal by C.M.O. quite obvious from the attached document to the grievance.
