The matter concerns the communication dated 23rd March 2023 of the chief medical officer District-Mirzapur who is procrastinating on the endorsed direction of D.M. Mirzapur in providing information concerning five point information because of corruption. Annexure to this grievance are
1-Two pages of communication dated 23rd March 2023
2-Three pages are the enquiry report.
3-Rest of pages are details of the report.
The Name and details of the wife of the applicant was also available in the enquiry report.
श्रीमान मुख्य चिकित्सा अधिकारी महोदय जनपद मिर्जापुर प्रार्थी को अत्यंत खुशी का अनुभव हो रहा है कि आप प्रार्थी को सूचना देना चाहते हैं किंतु प्रार्थी के औरत का नाम आपको मालूम नहीं है तो श्रीमान जी प्रार्थी के औरत का नाम रूबी पति का नाम प्रीतेश कुमार मोहल्ला डऺगहर पथरहिया जनपद मिर्जापुर है प्रार्थी का पिन कोड 231001
श्रीमान जी आप द्वारा निम्नलिखित पत्र प्रार्थी को पंजीकृत डाक से भेजा गया है जिसमें आप द्वारा मांगी गई सूचना मैं इस चीज की असमर्थता व्यक्त की गई है कि सूचना चाहने वाले ने अपनी पत्नी का नाम नहीं दिया है इसलिए उक्त पत्र के क्रम में प्रार्थी द्वारा यह ग्रीवेंस प्रस्तुत किया जा रहा है जिसमें प्रार्थी द्वारा आपके द्वारा भेजे गए पत्र और उसके संलग्नक को स्कैन पेज के रूप में पीडीएफ फाइल बनाकर संलग्न कर दिया गया है

State Public Information Officer/ chief medical officer Mirzapur may provide the following information to the information seeker as follows.
1-According to available X-Ray report, delivery was to take place in month of June 2022 and your three doctors in the report found the patient in labour pain in the beginning of the month of April 2022. Please provide the test carried out by the three doctors as mentioned in the report or any other thing on which findings are based.
2-Please provide the copy of consent of the information seeker in which information seeker opposed process of surgery so that applicant may know when they managed to get sign of the applicant and how?
3-Please provide the discharge report made on 16 April 2022 when patient was first discharged, and time and name of staff carried out process as well.
4-Please provide the time when patient was readmitted and name of staff who processed it and time span between discharge and readmission.
5- Please provide the discharge report made on 16 April 2022 finally after the expire of the patient.

Undoubtedly the information sought by the information seeker concerns the working of both the police and the department of medical and health unfortunately both are escaping from the matter because of the corruption. Undoubtedly the information comes under the ambit of right to information act 2005 but who had provided the information both of them denied the information arbitrarily and here information seeker is not seeking information but the action against the wrongdoer. This matter also concerns the cold-blooded murder of the applicant, and the applicant wants to expose them, but it is unfortunate that criminals are escaping from the prosecution because of their influence. Here I am not seeking information but the action against the wrongdoers. Here motive of the applicant has been fulfilled because concerned accountable staff read about the matter whether he is acting or not it depends upon the working style of the government.
