* Description of Information Sought The licences to the fair price shop owners shall be issued keeping in view the visibility of the fair price. Fair Price Shop owners who do not conform will be strictly punished as per rule. The licensing authority shall ensure the proper verification through Block Supply officer/Supply Inspector and shall recommend its report to the District Level Selection/Sanction Committee for consideration. The matter concerns fair price shop license holder Panchu Pasi in Village panchayat Adampur, Block-Chhanvey, District-Mirzapur, PIN Code-231303. There are 918 Fair Price Shops in the rural area of the District-Mirzapur. One Fair Price Shop is suspended in less than one month and four Fair Price Shops are suspended for more than two months consequently the total number of suspended Fair Price Shops is five in rural areas. One Fair Price Shop is cancelled in less than one month, eight Fair Price Shops are cancelled between one and two months and thirteen Fair Price Shops are cancelled for more than two months consequently the total number of cancelled Fair Price Shops is five22 in rural areas. In the rural area there are 27 attached Fair Price Shops and 22 vacant shops quite obvious from the second page of the attached PDF document to this application.

1-Herewith attached five-page communications showing pecuniary penalty Rs.1000 imposed by S.D.M. Gaurav Shrivastava and this penalty was made treasury challan in the state bank of India in appropriate head quite obvious from the first page.
Please provide the name of current License holder of Fair Price Shops in village panchayat-Adampur and date when the license was issued by the department to License holder.
2-Please provide a copy of the license containing time of validity.
3-Provide the process of license renewal in regard to Fair Price Shops.
4-Please provide a circular which allows tainted license holders to apply for renewal in this case pecuniary penalty and its recovery itself prove default cum offence.
5-Provide the reason of not inviting applications for granting license for Fair Price Shops as in the rural area there are 27 attached Fair Price Shops and 22 vacant shops quite obvious from the second page of the attached PDF document to this application.
