Most respected district magistrate Mirzapur following communication was sent by Block Development Officer Halia Mirzapur in your service on 5th March 2024 which must be pursued by you. This report was submitted by Block Development Officer Halia Mirzapur in the grievance submitted by the applicant on 17th February 2024 which is quite obvious from the following communication.

Mrs. Sadhna Tiwari, wife of Omkar Tiwari, resident of Surekapuram Colony, Riva Road, Sangmohal Kotwali, Katra, Uttar Pradesh, please take reference of the online complaint portal number 60000240036275 dated 17-02-2024, in which she has informed about her father late Siyakant, son of Shambhu Sharan, resident of Gram Panchayat, Bhaisodvalaya Pahad, has requested to issue a copy of the family register by adding her name in the family register.
The case was investigated by Shri Saurabh Yadav, Gram Panchayat Officer, Development Block, Halia, in which he informed that the said complainant has given an affidavit and class 8 transfer certificate on stamp paper of Rs 10, on the basis of which, after holding the meeting of Gram Panchayat, name Sadhana Tiwari will be included in the family register and a copy of the family register will be made available to Sadhana Tiwari. The report is respectfully sent.
Block Development Officer Halia Mirzapur
Shri Saurabh Yadav, Gram Panchayat Officer, Development Block, Halia, took under teeth the provisions of law because he repeatedly made calls on the mobile number of the applicant and called the applicant on the Bhaisod office and when the applicant sent her guardian at the Bhaisod office, then he always remains absent which reflects dereliction of duty and insolence to the applicant which is tantamount to harassment of applicant so his accountability must be fixed.
It is quite obvious that numerous representations submitted by the applicant but entire representations of the applicant gone in vain because of the Lackadaisical approach of the concerned secretary of the village panchayat. It is quite obvious that he made the mockery of the law ridiculously through his false assurances. Undoubtedly his dealings are lowering the dignity of public office which is quite obvious from his cryptic and mysterious dealings. Repeated communication addressed to the district magistrate Mirzapur of the block development Officer Haliya and not providing the proper solution to the applicant is only showing that Block Development Officer has no regard to the District- magistrate and he is only misleading the District Magistrate.

It is quite obvious that after one and half years block development Officer Halia provided the death certificate and family register to the applicant and it the applicant thanks to the concerned staff working in the office of block development Officer Haliya. Undoubtedly they made long delay in the matter but after all they provided the death certificate and family register so their act is praiseworthy and applicant thanks to them and wishes best of luck for them.

It is quite obvious that Block Development Officer Halia Mirzapur submitted following report before the district magistrate Mirzapur regarding the family register for the applicant.
Besides this he is procrastinating on the representation of the applicant submitted before him to add the name of the applicant in the family register and then provide the copy of the family register to the applicant which is quite obvious from the following communication addressed to the district magistrate Mirzapur by Block Development Officer Halia Mirzapur. Saurabh Yadav secretary of the village panchayat called the applicant in the said village panchayat on Monday but now he is telling to come on Friday such deliberate procrastinations are continued which are frustrating for the applicant.

Grievance Description-It is quite obvious that secretary of the village panchayat submitted arbitrary and inconsistent report and concerned block development officer of the development block Haliya accepted it without taking the perusal of it and blindly forwarded it to the district magistrate Mirzapur. Whether the office of the subdivisional magistrate Lalganj will include the name of the applicant in the family register of the deceased Siyakant Mishra S/O Shambhu Saran Mishra? It is quite obvious that family register is maintained under the ambit of block development officer and custodian of the family register is the secretary of the village panchayat. It is most ridiculous to escape from the matter by the secretary of the village panchayat because of the rampant corruption in the working.

Grievance Description-In a report dated 26 January 2024, Block Development Officer Halia told the applicant Family register in the name of Shambhu Sharan Mishra is not available. Therefore a copy of the family register of Shambhu Sharan Mishra can not be made available to the applicant. Please take a glance of the first page of the attached PDF document to the grievance.For the evidence of inheritance, the applicant requested the secretary of the village panchayat Mr Saurabh Yadav to provide the family register of the father by connecting the name of the applicant with the family register and he suggested the applicant to submit the representation with the affidavit on the stamp paper of Rs.10 addressed to the block development officer Haliya district- Mirzapur. Think about the gravity of situation the matter is more than one years old and it is quite obvious that in this matter not only block development officer Halia procrastinated on it but also the sub divisional magistrate Lalganj quiet obvious from the huge number of representations submitted by the applicant Sadhana tiwari. Most respected sir good governance implies that there is transparency and accountability in the working of the public authorities. Why are they procrastinated on the issue of providing the death certificate of the father of the aggrieved applicants Sadhana Tiwari? Now the chapter to providing the death certificate and family register have been over because they have provided it so there is no need to criticize them at present because it will not cultivate any positive result in this matter. I think that more than two dozen representations have been submitted by the applicant to get the death certificate and family register does not reflect good governance.
