Grievance Description
The matter concerns the working of the chief medical officer district Mirzapur related with the spray of anti-larva solution in Mohalla Surekapuram colony.
Short submissions of the applicant is as follows.
1-The applicant wants to remind you that if the anti-larva solution will not be sprayed in the Surekapuram colony then complaint will be made by the applicant.
2- It is quite obvious that no anti larva solution has been sprayed by concerned subordinates of the Filaria officer district Mirzapur which resulted this complaint.
3- Earlier filaria officer told to Chief Medical Officer district Mirzapur that they used to spray anti-larva solution in the Surekapuram colony per week. Unfortunately, they are not keeping their promise which reflects the dereliction of the concerned staff also the breach of trust which they promised to keep with the applicant under the mediation of Chief Medical Officer.
4- Now the applicant signs when they spray the anti-larva solution in Mohalla Surekapuram Colony and they keep the record safe for evidence quite obvious from the attached pdf documents to the grievance.
5- When the applicant is making allegations that the staff of the filaria officer district- Mirzapur is not spraying the anti-larva solution in Surekapuram colony for two months then it is their obligatory duty to attach the record of two months.
The population of the mosquitoes is rising alarmingly in Surekapuram colony so they must provide the report at the earliest and take action immediately so that a spray of larva solution could be made in the Surekapuram Colony.

The subordinates of the filaria officer submitted on the portal inconsistent and bogus report. These reports are attached to the grievance for the perusal of the most respected sir. Following is the feedback.
प्राप्त फीडबैक -दिनांक03-03-2023 को फीडबैक:-मुख्य चिकित्सा अधिकारी महोदय यह सच है कि मैं उन लोगों के कहने पर हस्ताक्षर नहीं करता किंतु उनके द्वारा एक मोबाइल फोन पर आपसे बातचीत कराई गई उस बातचीत में सामने वाला व्यक्ति मुख्य चिकित्सा अधिकारी के रूप में हमसे बात किया किंतु इस रिपोर्ट्स से यह स्पष्ट है कि आपने हमसे बात नहीं किया है उसके पश्चात प्रार्थी द्वारा यह डिसाइड किया गया कि अब हर बार प्रार्थी द्वारा हस्ताक्षर किया जाएगा और यदि किसी बार हस्ताक्षर नहीं किया गया तो इसका मतलब यह समझा जाएगा कि इन लोगों ने छिड़काव नहीं किया है और जिस हफ्ते ए लोग छिड़काव नहीं करेंगे उसी हफ्ते प्रार्थना पत्र दिया जाएगा अब उस स्थित के लिए इन्हें तैयार रहना चाहिए क्योंकि यह कहते हैं कि हर हफ्ते मोहल्ले में छिड़काव कर आते हैं तो जिस हफ्ते में भी नहीं आएंगे उस हफ्ते में शिकायत आपके यहां पहुंच जाएगा आप निश्चिंत रहिए सुशासन नियमानुसार कार्य यह हमारे सिद्धांत के लक्ष्य हैं और लक्ष्य को हासिल करना ही मेरा उद्देश्य है श्रीमान जी यदि मेरे हस्ताक्षर से इन लोगों की कार्यशैली में सुधार हो तो मैं 24 घंटे प्रस्तुत हूं मेरा उद्देश्य ही जनता की सेवा है और भ्रष्टाचार का विरोध है सरकारी सुविधाएं जनता तक पहुंचे यही मेरा लक्ष्य है सेवा में मुख्य चिकित्सा अधिकारी मीरजापुर शिकायत सं0 40019823003753 दिनांक - 01.03.2023 उपरोक्त आइ.जी. आर.एस के शिकायत के सम्बंध में अवगत करना है कि मुहल्ला सुरेकापुरम में अधोहस्ताक्षरी के कार्यालय में कार्यरत कर्मचारियों द्वारा दिनांक 12.01.2023 से 16.2.2023 तक शिकायत के मूहल्ला सुरेका पुरम में कुल 5 बार दिनाक 12.1.2023 19.01. 2023 02.02.2023 9.02.2023 16.02.2023 छिड़काव रिपोर्ट भेजा गया था। परन्तु इनके द्वारा आरोप लगाया गया कि यह प्रमाणपत्र फर्जी तरीके से तैयार किया गया है जबकि कर्मचारियों के जाने पर इनके द्वारा छिड़काव किये गये प्रमाणपत्र पर हस्ताक्षर नहीं किया जाता था। दिनांक 28.02.2023 को फाइलेरिया नियंत्रण इकाई के कर्मचारियों द्वारा दिये गये प्रमाणपत्र में बहुत कहने पर इन्होने हस्ताक्षर किया। उक्त रिपोर्ट की छाया प्रति लगाकर शिकायत का निवारण हेतु आपकी सेवा में प्रस्तुत किया जा रहा है। मुख्य चिकित्साधिकारी मीरजापुर दिनेश फाइलेरिया नियंत्रण अधिकारी मीरजापुर

Undoubtedly concerned staff of the Department of malaria did spray of anti-larva solution after the submission of the grievance on the public grievance portal while they had to spread the until Arba solution as their own duty. I think that clean and sanitation campaign is the part of good health of the citizens in the state so it is the obligatory duty of the concerned staff of the Department of medical and health to perform their duties timely without any pressure on them. Our political masters claim to provide good governance in good governance there is transparency and accountability in the working of the public authorities but where is accountability if no one is providing the reason why had they not sprayed anti-larva solution in the colony for three months? When the complaint is made against them their staff come to the colony and spray anti-larva solution which has been daily routine of them, they are not doing their duties automatically. We need good governance.
