विषय-श्रीमान जी खंड विकास अधिकारी हलिया द्वारा बार-बार श्रीमान जिला अधिकारी महोदय को संबोधित अपने पत्र में यह कहा गया की प्रार्थिनी द्वारा कोई भी आवेदन मृत्यु प्रमाण पत्र वास्ते नहीं दिया गया और प्रार्थिनी द्वारा 20 जनवरी 2023 को जो आवेदन दिया गया था हलफनामे के साथ उसमे यह कहा गया कि वह आवेदन खंड विकास अधिकारी हालिया तक पहुंचा ही नहीं उसी क्रम में प्रार्थिनी द्वारा कल अर्थात शुक्रवार को 22 दिसंबर 2023 अपने चाचा के माध्यम से आवेदन हाथों-हाथ खंड विकास अधिकारी हलिया के क्षेत्रीय कार्यालय भैसोड़ पर उपलब्ध कराया गया उसके साथ 10 पन्नों का संलग्नक भी लगाया गया है वह संलग्नक प्रार्थिनी द्वारा कार्यालय के कर्मचारी संतोष के मोबाइल नंबर 9685283574 पर भेजा गया उनके द्वारा आश्वासन दिया गया था कि आज प्रार्थिनी को मृत्यु प्रमाण पत्र और परिवार रजिस्टर की नकल उपलब्ध करा दी जाएगी किंतु आज सुबह मोबाइल पर वार्ता करने के उपरांत उन्होंने प्राथमिक को यह बताया की सक्षम अधिकारी जिनको परिवार रजिस्टर और मृत्यु प्रमाण पत्र पर हस्ताक्षर करना है वह 25 दिनों से कार्यालय नहीं आ रहे हैं इसलिए उनका मृत्यु प्रमाण पत्र और परिवार रजिस्टर की नकल जारी नहीं हो सकता है उसके लिए प्रार्थना को सक्षम अधिकारी के कार्यालय आने तक का इंतजार करना होगा Grievance Status for registration number GOVUPE20230084523 Grievance Concerns To Name Of Complainant Sadhana Tiwari Date of Receipt 20122023 Received By Ministry/Department-Uttar Pradesh श्रीमान जी कृपया सक्षम अधिकारी जिनके माध्यम से मृत्यु प्रमाण पत्र जारी होता है परिवार रजिस्टर की नकल प्राप्त होती है उनका नाम व पोस्ट प्रार्थिनी को उपलब्ध कराये श्रीमान जी यदि उक्त अधिकारी द्वारा कोई मेडिकल रिपोर्ट सक्षम अधिकारी को उपलब्ध कराया गया है तो उक्त मेडिकल रिपोर्ट की कॉपी उपलब्ध कराये

Why did sub divisional magistrate tehsil Lalganj, district Mirzapur not forward application of the applicant for the death certificate of her father dated 20th January 2023 to the block development officer Haliya district Mirzapur? Whether it is not reflecting complete mismanagement and anarchy in the working of the sub divisional magistrate Lalganj district Mirzapur?
Sir, think about the gravity of the situation that in the month of January, the applicant applied for a death certificate, but it is being said by the village development officer that the applicant has not yet given any application to get the death certificate of her father.
The insensitivity of the staff of Lalganj Tehsil can be guessed from the fact that the application given by the applicant for obtaining the certificate was not sent to the office of concerned Block Development Officer.
Sir, is there no anarchy in the working style of the sub divisional magistrate Lalganj Tehsil where applications of the people are put in a dump of garbage, imagine our Chief Minister and our Prime Minister talking about the comforts of the people, but their subordinate officers put the problems of the public and the applications related to it in the heap of garbage.
Sir, such a system does not reflect good governance but reflects utter anarchy.
If there is corruption in the system, then such irregularities will be part of the dealing of the system and it is the moral responsibility of our Honourable Chief Minister and Honourable Prime Minister to improve the system.

Why did sub divisional magistrate tehsil Lalganj, district Mirzapur not forward application of the applicant for the death certificate of her father dated 20th January 2023 to the block development officer Haliya district Mirzapur? Most respected Sir, following is the report of the block development officer Haliya, district Mirzapur is again inviting the application for death certificate of the father of the applicant which is illegal because the applicant has already submitted the application for the death certificate for her father on 20th January 2023 with all relevant documents along with affidavit which is quite obvious from the attached PDF documents to the grievance. To invite multiple applications for the death certificate of the father of the applicant by the block development officer Halia district Mirzapur is not only illegal but also time consuming and a waste of time. प्रकरण में सम्बंधित से आख्या प्राप्त कर निस्तारण हेतु प्रेषित है आख्या संलग्न है आवेदक को अवगत कराना है मृत्यु प्रमाण पत्र हेतु सम्बंधित दस्तावेज ग्राम पंचायत के कार्यालय में प्रस्तुत करे जिससे शिकायत का निस्तारण समुचित ढंग से किया जा सके l

Grievance Description-Why did sub divisional magistrate tehsil Lalganj, district Mirzapur not forward application of the applicant for the death certificate of her father dated 20th January 2023 to the block development officer Haliya district Mirzapur? Whether it is not reflecting complete mismanagement and anarchy in the working of the sub divisional magistrate Lalganj district Mirzapur? The report of the assistant development officer development block Halia district Mirzapur submitted on the public grievance portal of the Government of India is also attached as the sixth page of the attached PDF documents to the grievance.
In its report, the block development officer is saying that all concerning documents for the death certificate must be submitted in the office of the gram panchayat so that the complaint may be suitably redressed. It is ridiculous to ask for concerning documents in support of application and next application with affidavit for the death certificate of father of the applicant while the applicant has already submitted the application for the death certificate with all relevant documents along with affidavit in the office of sub divisional magistrate Lalganj which was forwarded to the block development officer Halia. Because of rampant corruption in the working of the block development officer his subordinates did not entertain the forwarded communication of the applicant which was forwarded by the office of the sub divisional magistrate Lalganj. It is quite obvious that office bearers of the sub divisional magistrate Lalganj are mute on the matter and the reports are being submitted by the staff of the block development officer Halia which are arbitrary and inconsistent to the contents of the grievance.
