Why did sub divisional magistrate tehsil Lalganj, district Mirzapur not forward application of the applicant for the death certificate of her father dated 20th January 2023 to the block development officer Haliya district Mirzapur? Whether it is not reflecting complete mismanagement and anarchy in the working of the sub divisional magistrate Lalganj district Mirzapur? The report of the assistant development officer development block Halia district Mirzapur submitted on the public grievance portal of the Government of India is also attached as the first page of the attached PDF documents to the grievance.
From the following inquiry it is quite obvious that representation for the death certificate was submitted by the applicant on 20th January 2023 before the sub divisional magistrate Lalganj and it is expected from the sub divisional magistrate Lalganj to forward the representation of the applicant to the concerned mandate but it is most unfortunate that concerned assistant development officer of the development block Halia denying the application of the applicant so it is obligatory duty of the sub divisional magistrate Lalganj to ensure the reach after representation of the applicant to the A.D.O. panchayat Haliya district Mirzapur. Whether it is not reflecting anarchy in the working of the sub divisional magistrate Lalganj where representations of the aggrieved people thrown into the dustbin quite obvious from the report of the officer submitted on the public grievance portal of the Government of India.
1-Please provide the status of the representation made by the applicant before the sub divisional magistrate Lalganj, district Mirzapur, dated 20th January 2023.
2-Right to reason is the indispensable part of sound administrative system therefore if no action taken by the sub divisional magistrate Lalganj on the representation of the applicant then provide the reason of it to the applicant.
3-Sub divisional magistrate Lalganj may provide the minutes of proceedings to the applicant regarding the application made by the applicant before him as mentioned above.
If the matter was not forwarded by the sub divisional magistrate Lalganj to the block development officer Halia then it is fault of the sub divisional magistrate Lalganj because he is procrastinating on the representation of the applicant belongs to the vulnerable section.

It is most unfortunate that sub divisional magistrate tehsil Lalganj district Mirzapur Uttar Pradesh could not provide death certificate of my deceased father in 8 months even when the applicant completed entire formalities and they make various pretexts in the name of providing death certificate to the applicant.
The matter concerns the working of the sub divisional magistrate tehsil Lalganj district Mirzapur Uttar Pradesh. The following diseased is the father of the applicant whose address is as follows.
Siya Kant Mishra S/O Shambhusaran Mishra, village- Bhaisod Balay Pahad post-Daramalganj, tehsil-Lalganj, development block-Halia, District- Mirzapur Uttar Pradesh.
This application is under Article 51 A of the constitution of India to seek inquiry regarding the application made by the applicant before the sub divisional magistrate Lalganj dated 20th January 2023 with stamp of Rs.10 verified by the notary for the death certificate of my father Siya Kant Mishra S/O Late Shambhusaran Mishra which is attached as page 4 of the attached PDF documents to the grievance.
Honourable Sir may be pleased to take the perusal off the page 4 of the attached PDF documents to the grievance which contains 6 pages.
The first page of the attached PDF document is the transfer certificate of the applicant of the 8th standard and the second page is the mark sheet of the applicant of the 8th standard.
The 3rd page of the attached PDF document is the scan copy of the Aadhaar card of the applicant.
The 4th page of the attached PDF document is the application of the applicant for the death certificate of the father as mentioned above while 5th page of the attached PDF document is the non-judicial stamp verified by the notary to get the death certificate of the father.
The 6th page of the attached PDF document is the revenue record of the land which is showing that inheritance of the property in the name of my grandfather inherited to the 3 brothers of my father and his mother unfortunately there is no name my father in the revenue record.
The brief submissions of the applicant is as follows which must be taken into account by the concerned accountable staff of the tehsil Lalganj district Mirzapur Uttar Pradesh.
1-Please provide the status of the representation made by the applicant before the sub divisional magistrate Lalganj, district Mirzapur, dated 20th January 2023.
2-Right to reason is the indispensable part of sound administrative system therefore if no action taken by the sub divisional magistrate Lalganj on the representation of the applicant then provide the reason of it to the applicant.
3-Sub divisional magistrate Lalganj may provide the minutes of proceedings to the applicant regarding the application made by the applicant before him as mentioned above.
