प्रार्थी के औरत का नाम रूबी पति अर्थात प्रार्थी का नाम प्रीतेश कुमार, रूबी की उम्र २८ वर्ष दिनांक १६ अप्रैल २०२२ को जिला महिला चिकित्सालय में भर्ती कराया गया

महोदय जांच टीम द्वारा अपनी आंख्या में महिला अस्पताल के भ्रष्ट डॉक्टरों को छुपाने के लिए जिस प्राइवेट हॉस्पिटल का जिक्र किया है वह कोई और नहीं खुद महिला हॉस्पिटल के अधीन काम करने वाला महिला हॉस्पिटल का एमसीएच विंग है जोकि पब्लिक प्राइवेट पार्टनरशिप के थिअरी पर आधारित है जो हमारे महान प्रधानमंत्री और प्रदेश के मुखिया श्री योगी आदित्यनाथ के दूरदर्शिता पूर्ण सोच पर आधारित है
महोदय आप उनसे पूछिए कि उन्होंने प्रार्थी के पत्नी रूबी का क्या इलाज किया क्योंकि आपका कहना है कि जब प्रार्थी की पत्नी प्राइवेट हॉस्पिटल से लौटी तो उसकी हालत और भी खराब थी तो इस प्राइवेट हॉस्पिटल के द्वारा हमारी पत्नी रूबी का क्या इलाज किया गया हमारी पत्नी इस प्राइवेट हॉस्पिटल में कितने समय तक रही इन लोगों ने हमारी पत्नी का इलाज क्यों नहीं किया क्योंकि यह लोग बिना भारी-भरकम फीस लिए तो इलाज करते ही नहीं है और प्रार्थी की क्षमता थी ही नहीं प्रार्थी के पत्नी को इन्होंने को एडमिट किया ही नहीं बल्कि उन्होंने कुछ फार्म भरवाए थे प्रार्थी से किंतु इलाज नहीं किया तो इस पर उनसे रिपोर्ट मांगे और विवरण के लिए जांच टीम की जांच आख्या और एमसीएच विंग की काली करतूत संलग्नक के रूप में अटैच किया गया है और इस मैटर को प्रार्थी द्वारा उत्तर प्रदेश ह्यूमन राइट्स कमीशन में भी उठाया गया है
सन्दर्भ सं0 60000230083200 - आख्या आवेदक का नाम मो. 6393878790 श्री प्रितेश कुमार आवेदक द्वारा पोर्टल पर दर्ज महानिदेशक, चिकित्सा एवं स्वास्थ्य सेवाएं, उ० प्र0 महोदय के द्वारा दिनांक 22.05.2023 को मुख्य चिकित्साधिकारी, मीरजापुर के पोर्टल पर अंतरित की गई है, उक्त के सम्बन्ध में कहना है कि आवेदक द्वारा प्रेषित शिकायत जिला महिला चिकित्सालय (माँ विन्ध्यवासिनी स्वशासी राजकीय चिकित्सा महाविद्यालय, मीरजापुर) से सम्बन्धित है। जिला महिला चिकित्सालय के नियंत्रणाधीन अधिकारी प्रधानाचार्य, माँ विन्ध्यवासिनी स्वशासी राजकीय चिकित्सा महाविद्यालय, मीरजापुर है। जिला महिला चिकित्सालय वर्तमान में चिकित्सा शिक्षा अनुभाग के तहत संचालित है। अतः उक्त शिकायत का निस्तारण मुख्य चिकित्सा अधीक्षक, जिला महिला चिकित्सालय एवं प्रधानाचार्य, मॉ विन्ध्यासिनी स्वशासी राज्य चिकित्सा महाविद्यालय के स्तर से किया जायेगा
1-According to available X-Ray report, delivery was to take place in month of June 2022 and your three doctors in the report found the patient in labour pain in the beginning of the month of April 2022. Please provide the test carried out by the three doctors as mentioned in the report or any other thing on which findings are based.
2-Please provide the copy of consent of the information seeker in which information seeker opposed process of surgery so that applicant may know when they managed to get sign of the applicant and how?
3-Please provide the discharge report made on 16 April 2022 when the patient was first discharged, and time and name of staff carried out process as well.
4-Please provide the time when the patient was readmitted and the name of the staff who processed it and the time span between discharge and readmission.

In the following report, chief medical officer Mirzapur itself accepting that the matter concerns the working of MCH wing of government women hospital which works under the ambit of the principal of Maa Vindhyavasini medical college so the matter can be redressed by the principal of Maa Vindhyavasini medical college so they had to transfer it to concerned mandate for redress. In order of the above, it is to be informed that the applicant's wife Mrs. Ruby and the new born baby were treated in the District Women's Hospital, Mirzapur and the District Women's Hospital, Mirzapur is under the control of Maa Vindhyavasini Autonomous Government Medical College, Mirzapur. The applicant can obtain the desired information by contacting the Principal, Maa Vindhyawasini Autonomous State Medical College / Chief Medical Superintendent, District Women's Hospital, Mirzapur.
