* Description of Information Sought This online RTI application under subsection 1 of section 6 of the right to information act 2005 is on behalf of Mr Jaychand Maurya S/O Mr Rajaram Maurya village- Manauvan, Post- Khamariya, police station- Chilh, Tehsil-Sadar Pargana Kantit, district- Mirzapur, Uttar Pradesh, mobile number- 8090306360.
The description of the attached pdf document to this online application is as follows.
1- The first page of the attached pdf document is the representation of the applicant Mr Jaychand Maurya dated-23 September 2024 before additional director medical and health division Vindhyachal, Mirzapur for two original papers taken from the applicant during the course of inquiry.
2- The second page of the attached PDF Document is the medical slip provided by Dr Pradeep Kumar Yadav during the private practise in Chhotelal Hospital G.T. Road,Baraut, Prayagraj Uttar Pradesh and the wife of the applicant was undergoing treatment in his monitoring. The original paper of this slip was taken by the office of additional director medical and health during the processing of inquiry with the assurance that they will return it.This original paper is under the protection of Office of additional director medical and health since 5th August 2024.
3-The third page of the attached PDF Document is the medical slip provided by Dr Pradeep Kumar Yadav during the private practise in Chhotelal Hospital G.T. Road,Baraut, Prayagraj Uttar Pradesh and the patient Abhay Raj Bishwakarma was undergoing treatment in his monitoring. The original paper of this slip was taken by the office of additional director medical and health during the processing of inquiry with the assurance that they will return it. This original paper is under the protection of Office of additional director medical and health since 5th August 2024.
4- The 4th page is the affidavit of the aggravated applicant Mr Jaychand Maurya S/O Mr Rajaram Maurya authorising information seeker to seek information.
Public information officer must provide the following information point wise as follows.
1- Please provide the notings and action taken on the representation of the aggrieved applicant Mr Jaychand Maurya S/O Mr Rajaram Maurya dated-23 September 2024 before additional director medical and health.
2- Please provide the reason to deny the two original papers mentioned under point two and three in document description quite obvious the applicant wrote representation dated 23rd September 2024.
3- Please provide the name and designation of the staff under whose custody these two original papers are protected.
4- If the aforementioned original papers are missing then provide the copy of the FIR in the matter if FIR was not registered then provide the reason of it.
5- Now inquiry has been over and enquiry report has been submitted then what is the need to keep the original papers as the applicant to keep under his possession by the office of additional director of medical and health. Provide the reason of criminal breach of trust by the office of the additional director.
The public information officer must provide the five points information to the information seeker within a stipulated 30 days as prescribed under subsection 1 of section 7 of the right to information act 2005.
