Sir, applicant submitted following R.T.I. Request on the RTI online portal of the government of Uttar Pradesh. Unfortunately portal did not uploaded the required PDF document so please forward the grievance to chief medical officer Mirzapur so that he may not take recourse to procrastinate that concerned document not attached to the RTI Application.

Mirzapur and second page of the attached document is the enquiry report submitted before Uttar Pradesh Human Rights Commission Lucknow under the signature of 1-Dr. U.N. Singh, District Tuberculosis officer, Mirzapur, 2- C.O. City, Prabhat Rai, 3-Dr. Kiran Kala, senior specialist of women diseases and 4-Dr Vivek Singh, Child disease specialist S.N.C.U. Woman hospital Mirzapur.
State Public Information Officer/ chief medical officer Mirzapur may provide the following information to the information seeker as follows.
1-According to available X-Ray report, delivery was to take place in month of June 2022 and your three doctors in the report found the patient in labour pain in the beginning of the month of April 2022.
Please provide the test carried out by the three doctors as mentioned in the report or any other thing on which findings are based.
2-Please provide the copy of consent of the information seeker in which information seeker opposed process of surgery so that applicant may know when they managed to get sign of the applicant and how?
3-Please provide the discharge report made on 16 April 2022 when patient was first discharged, and time and name of staff carried out process as well.
4-Please provide the time when patient was readmitted and name of staff who processed it and time span between discharge and readmission.
5- Please provide the discharge report made on 16 April 2022 finally after the expire of the patient.
After all it is obligatory duty of the care takers of patients to take discharge report of the patients, but nothing was provided by the women hospital Mirzapur.
Please provide aforementioned five points information to the information seeker so that commission may be apprised with the factual position of the case.

It is quite obvious that circle officer city always submits his report like a parrot and never changes the contents of the report whatever may be the contents of the submitted grievance. It is crystal clear that the wife of Pritesh Kumar died because of criminal mismanagement in the working of the staff of the women-hospital. The fault of the police is that it supported the defaulter staff of the women hospital Mirzapur city. Now it has been clear that cold blooded murder of the wife of Pritesh Kumar belongs oppressed section has been put under the carpet. There is no justice for the weaker and downtrodden section in this largest democracy in the world. Now the applicant has been frustrated because of bad governance in the state of Uttar Pradesh in which poor people are no more safe. Whatever scanned copy of the report has been submitted is quite illegible. Modi Sir, you joked Akhilesh Government in the name of Hakrana, Makrana and Shukrana but did not speak single world against it
